Getting renewed prescriptions is a mess.
Providers are excellent and support is responsive and helpful but literally every renewal of what is intended as a continuing prescription is a total failure. I used Noom previously and never had to manually reorder or deal with problems getting my prescription - it was delivered to my door on a known schedule. I re ordered and my provider approved promptly and yet I’ve already missed one dose and have no idea when I’ll receive my prescription.

I love Form.
I love Form. I have never been this successful or this healthy in my adult life. I initially lost 75lbs taking no higher than 5mg of tirzepatide with making significant nutritional changes, and adding weight lifting. In the past, I have tried WW and medifast prior and never lost more than 20lbs, and always put it back on and then some, until I was desperate for help at 231lbs and 5'3". Bariatric surgery was never an option for me. With form I did put 15lbs back on, but overall I am down 60+lbs for over 2 years, maintaining around 165lbs. This is a very good program, very supportive, and very easy to track your weight and eating. They send you a scale that sends updates to them through cellular so you don't have to worry about tracking. I would give form 5 stars, for my Dietitian, Erika, especially. However, I am now onto my 4th doctor since 2022. That bugs me, because I have to repeatedly tell my story over and over and lose any continuity of care. I also think they are using AI for messaging with the care coordinator. I will continue to be a Form user for life.

Form Health Gives Me Confidence and Security
I’m so grateful to have Form Health as my guidance and support system as I navigate this important weight loss journey. I was nervous to begin the injections- worried about side effects and had so many questions about the entire process. With Form Health, you have a Care Team comprised of a Doctor, a Nurse Practitioner, a Nutritionist and a Care Coordinator. They are with me every step of the way. I have a monthly zoom call with either the doctor or the nurse practitioner and the nutritionist once a month- though I can reach out via the app ANY time I have a question or problem and the response time has always been quick. They monitor my progress and my health. I feel beyond grateful to be a part of Form Health and know I have wonderful, trusted professionals helping me every step of the way.

Been a great experience!
Both Dr. Yen and Julia, my nutritionist are great to work with. I have dropped over 25 lbs since joining the program and hope to lose another 15-20. The cost to me has been zero since Medicare has paid the cost.
